Citation: At approximately 2000 hours I was leaving the building and noticed a sufficient amount of water coming out from the bottom of the door at the offal room; also water was coming out from under two garage doors; water with chicken particles in the water. Upon looking in the offal room I found four to five inches of water throughout the entire room. It appears that two different pipes have water pouring out of them. I immediately notified the Lead in charge of the situation. Maintenance is now taking corrective actions of the situation. Continued failure to meet the regulatory requirements can lead to enforcement actions as described in 9 CFR part 500.4 Past Similar NRs - Previous Ineffective Plant Actions: A continued backup in sewer and water pipes. No adequate floor drainage in areas where floors are subject to flooding-type cleaning. NR: 89-2011 dated 11/8/2011; NR: 86-2011 dated 10/20/2011
