Citation: On reviewing the water chlorine testing records on 02/08/2011 as part of an unscheduled 03J01 procedure, I noted documentation of high total chlorine levels at the reprocessing station. The recording, made at 0817 hrs, was documented as 110ppm. As per the corrective actions, production at reprocessing and salvage stations was stopped and maintenance notified. A retest performed at 0827 hrs was within normal limits and production was continued. The last acceptable test was performed at the reprocessing station at 0753 hrs ( ppm) and at the salvage station at 0752 hrs. No product was retained. Reprocessed and salvage product produced between the last acceptable test and the high recorded test measure is considered not acceptable due to potential exposure to chlorine levels in excess of The establishment addresses chlorine level at reprocessing and salvage stations as a control point in their pre-requisite program to support why excess chlorine is a hazard unlikely to occur. However, the program has not prevented the adulteration of product. The establishment failed to recognize the unforeseen hazard and did not follow the appropriate corrective actions including; 1. Segregation and holding all affected product, 2. Performing a review to assess the acceptability of the affected product for distribution, 3. Taking action with respect to the affected product to ensure that no product is adulterated enters commerce, and 4. Performance of a reassessment of the HACCP plan. This is a violation of 9 CFR 417.3(b)(1-4).


417.3(b)(1) Segregate and hold the affected product, at least until the requirements of paragraphs (b)(2) and (b)(3) of this section are met;

417.3(b)(2) Perform a review to determine the acceptability of the affected product for distribution

417.3(b)(3) Take action, when necessary, with respect to the affected product to ensure that no product that is injurious to health or otherwise adulterated, as a result of the deviation, enters commerce;

417.3(b)(4) Perform or obtain reassessment by an individual trained in accordance with 417.7 of this part, to determine whether the newly identified deviation or other unforeseen hazard should be incorporated into the HACCP plan.
