Citation: On 5/11/11, at approximately 08:10am, while performing unscheduled PBIS task code 06B01, I observed the following noncompliance in the custom/retail cooler. 1. The stainless steel table directly under the refrigeration unit, was seen to be covered in blood stains and numerous particles of dried ground beef, from many past days of production. 2. The fan guards on the refrigeration unit are in need of a general cleaning. 3. 3 hog rollers have a heavy build up of product residue. 4. 2 plastic storage racks have dried bloodstains and also are in need of cleaning. 5. The 3" piping used to hang hooks of inspected product, had rust on the vertical piping. 6. Smokehouse sticks were being stored on the floor, in addition to the smokehouse rack having black specs on 2-3 areas of the rack. No product was being stored on this truck at this time. 7. Walls were seen to have bloodstains and are also in need of a general cleaning. The items stated above were non-product contact zones, but this cooler looks like it has not had any type of general cleaning, in some time. Mr. [redacted] was informed of these deficiencies, both verbally and in writing with this NR, that this is in violation of Regulations 303.1(a)(2)(i);416.4(b);416.4(d)


416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(b)(1) Construction. Establishment buildings, including their structures, rooms, and compartments must be of sound construction, be kept in good repair, and be of sufficient size to allow for processing, handling, and storage of product in a manner that does not result in product adulteration or the creation of insanitary conditions.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.

303.1(a)(2)(i) Establishments that conduct custom operations must be maintained and operated in accordance with the provisions of 416.1 through 416.6, except for: 416.2(g)(2) through (6) of this chapter, regarding water reuse and any provisions of part 416 of this chapter relating to inspection or supervision of specified activities or other action by a Program employee. If custom operations are conducted in an official establishment, however, all of the provisions of Part 416 of this chapter of shall apply to those operations.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.4(d) Product must be protected from adulteration during processing, handling, storage, loading, and unloading at and during transportation from official establishments.
